What to Bring

 

When you prepare for the funeral arrangement conference where you will meet with your funeral director to finalize all arrangements for whatever services you wish to have, you will want to remember to bring along all necessary information and items the funeral director will need. 

 

Here at the Koller Funeral Home and Cremation Center, we have put together the following checklist to help you gather what will be required.  If your departed loved one has a complete end-of-life plan, that package may include almost everything you’ll need to bring.  So you might wish to compare especially the plan’s portion that relates to funeral / burial arrangements against the following checklist.

 

  • Full legal name and home address
  • Date and place of birth
  • Birth certificate
  • Social security number
  • Marriage certificate
  • Veteran’s discharge papers (DD214, if applicable)
  • Fathers name and mother’s full maiden name
  • Survivors’ full names, city/state, relationship
  • Highest education (schools, dates, degrees)
  • Occupation
  • Insurance policies (if applicable)
  • End-of-Life Plan package if available) including funeral/burial and documents related to pre-funding, if applicable
  • Clergy’s name and phone number
  • Photos
    • One recent
    • One favorite
    • Up to 30 favorites for memorial video (if applicable)
  • Clothing for the deceased, (outer garments, undergarments, socks/hosiery, jewelry, lapel pin, watch, rosary eyeglasses, dentures, other prosthetics)
